Professionally Designed Survey Templates

Creating a good survey takes time. You’re deciding on questions, picking scales, figuring out the right structure - it adds up. Starting today, you can skip that and go straight to collecting feedback.

What’s Included

We’ve added five templates for the most common feedback scenarios:

  • Net Promoter Score - The standard 0-10 loyalty scale. It automatically sorts responses into promoters, passives, and detractors.
  • Customer Satisfaction - Simple satisfaction surveys for products, services, or support. Quick to send, easy to read.
  • Customer Effort Score - Finds out how easy it was to complete a task. The 1-7 scale highlights where customers hit friction.
  • Team Health Check - Weekly pulse surveys built on validated psychometric research. You’ll spot engagement issues before they become problems.
  • Event Feedback - For conferences, trainings, and community events. Measures what worked, what didn’t, and the overall impact.

Each template page includes an article explaining why the questions were selected, the research behind them, how to interpret your results, and what to do next. Pick a template, tweak the questions if you need to, and share the link.

More Than Question Lists

Most survey tools offer templates that are just lists of questions. Ours are different - each template comes with a short methodology article that explains the research behind it.

For example, the Team Health Check uses the UWES-3 (Ultra-Short Work Engagement Scale), validated for weekly use. Its psychological safety item comes from Amy Edmondson’s research - the same framework Google highlighted in Project Aristotle when studying high-performing teams.

You don’t need to read the research to use the templates. But if you’ve ever wondered why we use “vigor” instead of “motivation,” or why the stress scale is 1-5 instead of 1-10, the reasoning is all there.

Built on What Works

Every template is grounded in established survey methods:

  • Scales that are properly balanced and validated
  • Clear, direct question wording
  • Logical ordering for smooth flow
  • Response anchors that reduce bias

What’s Next

We’re working on user accounts. Incognito surveys aren’t going anywhere - you’ll still be able to create and share surveys without signing up. But soon you’ll also have the option to create an account, save your work, and access additional features.
